Kotal might only be ruler of Outworld at some point in the game..

Just putting this out, since there's a lot of emphasis on the time specter of this game, 25 years, it might means that Kotal gets to be Emperor of outworld at some point in the time frame..

The story could easily have him overtaken by someone else over the lapse of 25 years.. This would open some place to have a returning main boss.

Just putting this out there... Wadayathink?

Definitely! I don't think him being emperor is about being a playable boss, so much as a story element.

Besides, they said we would be surprised by the final boss. So Kotal definitely isn't it. Can't really be surprised about a brand new character.
