Baraka is killed by D'vorah. There is a QTE where you perform the fatality, however his death is recognized even if you don't perform it.
Mileena is killed in the most awesomely gruesome way. After being defeated and captured, Kotal Kahn has D'Vorah execute her. I'll let you see it yourself, but it involves maggots ;) (apologies to Mileena fans).
Sektor is killed off-screen in the most anti-climatic way (most likely will be my biggest complaint of the story). Sub-Zero keeps his head on life support to show Scorpion his memories of dealing with Quan Chi.
Apparently Quan Chi is killed by a team-up of Scorpion and Subby Wubby. It sounds so satisfying I don't want to try and find it, just wait till the full thing is out.
Shinnok is defeated by Cassie Cage. Raiden later becomes dark Raiden and cuts off his head, but since he is a former Elder God he is left alive. Raiden makes him an example to warn Liu Kang and Kitana (the new rulers of the NetherRealm, how cool!) not to mess with him.
Raiden sacrifices himself to absorb the poison within the Jinsei (after Shinnok apparently successfully corrupted it). However he apparently survives and is seen at the very end as Dark Raiden. Apparently the poison he absorbed from the Jinsei corrupted him.
I've seen no evidence that shows that Jax dies. The last act of his chapter is called "Good to be back" and doesn't seem to imply he dies. Johnny is bound in some weird ropes and a little beaten up at the end, but he is alive and reunites with Sonya and Cassie. I didn't see Kenshi or Jax in the final scene. D'vorah seems to be alive as well, as Sonya arrives and commands her troops to isolate Shinnok and D'Vorah (after Shinnok's defeat), who are both knocked out.
Does anyone have a video or pictures of Mileena's death? That sounds nasty.
