ACCIDENT & FATALITY REPORT NO. 05632

Location: phazon mines ore processing aka fire well.

Type: FATALITY!!!

Report: crushed to death by ore crusher.

Details: space pirate was repairing a jam in the left ore crusher. He shut off the power to the safety shield and the machine but failed to lockout the power switch. Another one turned the power back on unaware of the that it was being repaired. Said he heard a scream of pain and a crunching noise. He ran and jumped down to investigate and discovered that the guy got crused and saw hs body parts get crushed one by one. Apparently the victem was unable to escape do to him being trapped behing the reactivated safety shield. Needless to say this got blood everyware.

Location: trooper armory phazite ore processing area aka the armory.

Type: FATALITY!!!

Report: Scorpion got crushed to death by large ore crusher while engaged in unautherized kombat.

Details: Taven was inspecting the area for safey hazards and violations when Scorpion attacked him. He fought him off and threw him against the barrier protecting the crusher and conveyor belt. However the safety glass on it was weak and shattered on impact. Scorpion was running away from the roller but stumbled and got crushed with a sickening crunch spraying blood everyware before Taven could trigger the emergancy shutdown. Now the roller has a bloody stain on it in the shape of a human. Access to the area is now restricted to mantainence personal only and the processing area will remain offline until the new safety glass is installed.

Location: Phazon mines large phazon ore crusher area. Aka upper & lower mines from mkd

Type: MULTIPLE FATALITYS!!!

Report: one space pirate got cut in half by mechanical dragon mouth while another one got grinded up in ore grinder.

Details: Due to the lack of safety barriers the deaths were not surprising. We are going to have them installed next week. The first one was due to stupidity on another pirates part. They were playing chicken with the mechanical head by sticking their hands into jaws while they were open. But one stumbled into them when they clamped down on his chest chopping him in half his screams did not last long but a loud crunch was heard and blood was splattered all over the place. Meanwhile in the the lower mine where the ore grinder is. Another one was standing near it when a rock fell and he jumped back into the roller and was pulled into it feet first. His screams of agony were echoing throuch the caves. As someone came down to investigate them he saw hin being pulled in and was splatterd with blood and saw dismembered bloody limbs and organs flying up behind it. The body parts were flying up for 10 min.

Type: FATALITY!!!

Report: Kenshi crushed to death between moving gears while engaged in unautherized kombat.

Details: The generator area has a safety shield in place above the moving synergy strut gears. These gears are exactly like the gears in Kahns mka arena. Kenshi was engaged in unautherized kombat with some other samurai warrior in the area. The other oppnents katana destroyed the safety shield control box causing the force field to shut down. The hazard alarms were tripped and security personal were en route to the room. However Kenshi was pushed into the exposed gears. There was a loud crunch and a yell of pain. He was caught in between them legs sticking out. The gears were jammed and the blood was spraying everywere as another crunch and another yell of pain. The gears finaly unjammed and a third yell of pain was heard as the legs were pulled in. Both gears were covered in blood. Access to the generator area is now restricted to prevent further accidents. And until the safety shield is back online no one will be allowed to inside unless its nessesary.

Type: injury

Status: crtitcal condition

Report: Sektor knocked into moving fan 3 times while engaged in unautherized kombat. Resulted in severe damage to the system as the 3 fans were completely destroyed.

Details: Taven was in the room when he ran into Sektor who attacked him because of his escape from the warship in mka konquest. During the fighting Sektor was knocked into the fan shattering the fan blades screaming as he was almost blended. He was then knocked through pipes with a bunch of oofs and bangs. He then crashed through the grating with an ouch. This happaned 3 times and caused the toxicity levels in sector C-3 to become so high that we had to seal off, isolate, and lockdown that sector. As of now the ventilation areas are off limits to everyone untill the replacement fans arrive. After that access will be restricted to autherized mantainance personal only.
