Credit goes to my friend Ken Sztorc for this.
As you may know, MK1 and MKII only support a 4-button gamepad. I'm not too sure about MK3 and such, but this will work for any PC MK where applicable.
First off, you will need a small application called JoyEmu. This basically emulates key strokes through your gamepad, and does it well. It's a bit difficult to configure, but I'll supply a basic tutorial on how to get it working. This was tested on Win98 and may or may not work on XP/2000.
First off, download the file using the link above and install it to a directory. Let's use C:\DOS\JOYEMU\ for our example. Once installed, add the following to your AUTOEXEC.BAT:
LH C:\DOS\JOYEMU\JOYEMU.EXE
LH C:\DOS\JOYEMU\JOYEMU.EXE JC:\DOS\JOYEMU\JOYSET.JS KC:\DOS\JOYEMU\KEYSET.KS
The AUTOEXEC.BAT entries are not necessary, but it offers the convenience of loading the application into memory automatically. Otherwise you will have to run the application manually or by creating a .BAT file. The only problem with loading it automatically is your gamepad will always be using the keys it is set to if loaded into memory.
The next step is to set your keys. So run JOYCP.EXE from C:\DOS\JOYEMU. Now pick Joy settings from the menu and enable Joystick 1. Configure it by adding the number of buttons you have and calibrating it. Now save the settings to JOYSET.JS. Go back to the menu and select Key settings. Make sure Hold mode is on and Force mode is off, go to Keys joystick 1 and set the keys you want your gamepad to use. Gamepads differ, but it works something like this:
Fire 1 A < keyboard letter
Fire 2 S
Fire 3 Q
Fire 4 W
Fire 5 D
Fire 6 E
JoyEmu can be ran one of two ways. Either by running JOYEM.EXE (without hotkeys) or JOYEMU.EXE (with hotkeys). I prefer running it with hotkeys. Hotkeys allow you to do things such as turn off/on the emulation, unload it from memory without using a syntax (IE: C:\DOS\JOYEMU\JOYEMU.EXE U to unload from memory), or adjust the sensibility of the emulation. See the included JOYEMU.TXT for a list of hotkeys and additional help.

I use JoyToKey to play MK1 and it works pretty well. Here's the link: http://hp.vector.co.jp/authors/VA016823/joytokey/english.html

There's nothing wrong with using the keyboard. I use it myself. But some people prefer a gamepad and the first two PC MKs (at least) only support a 4-button gamepad. MK is 5 buttons, obviously. I just offered a way to get around that.

Well not everyone is like you.
